Israel Nachfolger, RN, ASN, BS, NR-EMTP

Owner & CEO

Israel brings a unique mix of clinical and operational experience to Pinnacle, having worked in healthcare as both a practitioner and in various administration roles for over a decade. In addition to his role as CEO, Israel continues to practice as a volunteer Paramedic and manage multiple Skilled Nursing Facilities on the East Coast.

Israel lives in the foothills of the Ramapo Mountains in the Hudson Valley in Monsey, NY with his wife, Miriam, and 5 children. An avid lover of music, Israel enjoys honing his skills as a musician at the piano and practicing his passion as a recording engineer in his free time.

Mark A. Jacobs, LNHA, MSM, FACHCA


With his signature personal touch, Mark Jacobs brings many years of administration experience to Pinnacle Health & Rehab.

Having managed facilities in North Carolina, Georgia, and Tennessee earlier in his career, Mark has been at the helm of our facility for over 18 years. He worked alongside both his grandfather and our facility’s founder, Aiden Redding, and continues to maintain the caring and warm spirit his elders established.

Mark recently served as the distinguished president of the Maine Chapter of the American College of Health Care Administrators and maintains active participation in the Maine Health Care Association.

Mark graduated from Southern Adventist University with a BSBA and New England College with a MSM with an emphasis in health care administration.

Mark and his wife Valerie reside in neighboring Peru and is the treasurer and board member of the community church. They have four children, two grandchildren, one dog and four cats. Mark volunteers as the treasurer and board member for the community church.

Michele Hutchins, RN, BSN

Director of Nursing

Michelle has made nursing her life’s work since she started as a Certified Nurse’s Assistant over 25 years ago. Michelle’s caring and warm nature shines through in her work where she can be found during all times of day and night in our halls tending to our residents’ needs. In 2000, she joined our facility as a charge nurse, and has since worked in a variety of rolesincluding Admissions Coordinator, RN Supervisor and Assistant Director Of Nursing prior to assuming her current role as Director Of Nursing in 2006.

Michele graduated from the University of Maine in Augusta with an Associates degree in Nursing in 1999 and received a Bachelors of Nursing in 2016.

Michelle and her husband live in the nearby town of Dixfield and have 5 children and 5 grandchildren. She enjoys reading, camping, crocheting and sewing in her free time.

Shelly Desroches-Harlow, LPN

Residential Care Director

As our Residential Care Director, Shelly is our tour de force who seems to have endless energy to handle any task that comes her way. She has worked at our facility for over 23 years and considers it to be her second home, caring for our residents as if they are members of her own family. Michelle is well recognized for her kind demeanor and the unique personal connections she makes with both employees and residents.

Shelly received her degree as a Licensed Practical Nurse from New Hampshire Vocational Technical College in 1981, and became a Certified CPR instructor from the American Heart Association in 1983. More recently, Shelly received a Liberal Arts Degree from Central Maine Community College in 2016 and as a Mental Health Rehabilitation Technician from University of Southern Maine College in 2017 Shelly and her husband, Jeff, reside in Dixfield and have 3 children and 2 grandchildren

Naomi Marston, OTR/L

Director of Rehab Services

As the head of our dynamic Rehab department, Naomi brings an innate ability to bring out our residents’ full rehab potential in order to get them back to optimal functioning levels, quickly and safely. Her 19 years of experience in geriatric rehabilitation have given her a unique ability to overcome any challenges that come her way, with her goal always to enable our residents to regain their maximum level of independence. Naomi received a Bachelor’s Degree in Occupational Therapy from The University of New England in 2000.

In her free time, Naomi enjoys spending time with her husband Jason and her son along with their pets, in addition to camping, boating and hiking”.

Tanya Smith, LSW, MHRT-III

Social Services Director

Having graduated with a Bachelor of Science in Rehabilitation Services from University of Maine at Farmington in 2001, Tanya is the bridge from Pinnacle to the community for our residents. With more than a decade of experience at our facility, Tanya ensures that all of the needs of our residents and their families are achieved. She is also the main point of contact for our collaborative partnerships with area hospitals and doctors to ensure seamless continuity of care.

In her leisure time, Tanya enjoys reading and spending time with her daughter, Aiyana.

Becky Knox, ADC, CNA, CRMA

Director of Life Enrichment & Environmental Services

Serving as our Director of Life Enrichment & Environmental Services since 2012, Becky is known as the “life of the party” at Pinnacle. Her endless energy and enthusiasm are infectious to those around her, giving her the ability to spark the vitality of our residents and encourage participation.

During her free time, Becky enjoys spending time with her husband Kevin, reading and shopping for shoes.

Chris Bisson

Food Services Director

Chris began working in long-term care facilities almost ten years ago and found his passion was to provide the best meals possible for the residents. He’s happy that he’s found his life’s work and looks forward to providing meals that residents love and look forward to.

In his spare time, Chris loves to be outdoors enjoying nature as he hunts, fishes, hikes, and gardens. He is an avid canner of jellies, jams, pickles, relishes, and vegetables. His newest hobby is dehydrating new foods.

Melodie Abbott

Business Office Manager

As the Business Office Manager Melodie is the go to person for numerous tasks concerning payroll, resident trust and HR. She has been working in nursing home offices for over 30 years. This is not her first time in this facility, she did 26 years when it was the Victorian Villa and after a brief stint at another Maine facility has been a busy bee at Pinnacle since 2018.

In her spare time she likes reading, bird watching, gardening and canning her own vegetable and numerous kinds of pickles that her two children love to eat.
